Progressive metastatic pulmonary calcification after successful renal transplantation.

چکیده
Metastatic pulmonary calcification is a complication of patients with renal failure [1], but is rarely seen after successful renal transplantation. A 51-year-old Japanese male demonstrated progression of diffuse pulmonary ground-glass attenuations over time. He had been treated with oral corticosteroids and immunosuppressants since undergoing left renal transplantation from his mother at 26 years of age. A chest radiograph (fig. 1a and b) and high-resolution computed tomography (fig. 1c, d, e, g and h) showed diffuse centrilobular groundglass attenuations and partly high-density areas. These findings had slowly worsened over time. Tc-methylene diphosphonate (Tc-MDP) scintigraphy demonstrated high bilateral pulmonary uptake, especially in the upper and posterior lung fields (fig. 1f). This case is considered as continuously progressive metastatic pulmonary calcification after successful renal transplantation.

Metastatic pulmonary calcification after renal transplantation.
Metastatic pulmonary calcifications, unlike dystrophic calcifications, occur in the normal healthy lung. The radiological pattern is quite specific. The disease is commonly described in chronic renal failure with calcium disorders. The prognosis is totally unpredictable. In 1992, a 50 yr old man underwent a successful renal transplantation during the final stage of chronic renal failure. He sub...
متن کاملPulmonary metastatic calcification with respiratory insufficiency in patients on maintenance haemodialysis.
A uraemic patient undergoing chronic haemodialysis developed diffuse metastatic pulmonary calcification and died from acute respiratory insufficiency after renal transplantation. Thirteen similar cases previously published are reviewed, with emphasis on the clinical and anatomical features of such calcinosis. The pathogenesis of this calcification in patients on maintenance haemodialysis and so...
متن کاملPulmonary hydatid cyst and successful renal transplantation
Abstract Hydatid cysts are endemic hooknoses in Iran. It may involve various organs of body. Liver is involved in 80% and lung in 10- 15%of cases. 25% of pulmonary hydatid cyst is bilateral. Before any transplantations eradication of infection is necessary. In a 26-year old man with renal failure referred for kidney transplantation. On initial preoperative evaluation of this patient...
متن کاملMetastatic Pulmonary Calcinosis and Leukocytoclastic Vasculitis in a Patient with Multiple Myeloma
UNLABELLED Both leukocytoclastic vasculitis and metastatic pulmonary calcification are conditions that rarely occur during the course of multiple myeloma. We present a multiple myeloma patient that had severe dyspnea due to metastatic pulmonary calcinosis, and ulceronecrotic skin lesions caused by leukocytoclastic vasculitis. After 3 courses of standarddose chemotherapy all skin and pulmonary l...
متن کاملPulmonary Calciphylaxis Associated with Acute Respiratory and Renal Failure Due to Cryptogenic Hypercalcemia: An Autopsy Case Report
Metastatic calcification is rare; it is found during autopsy in patients who underwent hemodialysis. Diffuse calcium precipitation of small and medium-sized cutaneous vessels, known as calciphylaxis, can result in progressive tissue necrosis secondary to vascular calcification. This condition most commonly involves the skin; however, a rare occurrence of visceral calciphylaxis has been reported...
